Title: Mangelernährung bei Patienten mit Kopf-Hals-Tumor - Evaluation der Bioelektrischen Impedanzanalyse in der klinischen Praxis

Abstract: 
Hintergrund: Patienten mit Kopf-Hals-Tumoren sind aufgrund der unmittelbaren Betroffenheit ihrer oberen Verdauungsorgane durch Krankheitsgeschehen und Therapienebenwirkungen in besonderem Maße dem Risiko einer Mangelernährung ausgesetzt, wodurch die Prognose des Patienten negativ beeinflusst wird. Ein niedriger Phasenwinkel (PA) aus der Bioelektrischen Impedanzanalyse (BIA) ist häufig mit einem Ernährungsrisiko assoziiert.
In dieser Arbeit soll der PA aus der BIA als Tool in der Erkennung eines Ernährungsrisikos evaluiert werden. Methoden: Der PA von 30 onkologischen Patienten wurde vor Therapiebeginn bestimmt und die Patienten in Risikogruppen eingeteilt (NRS, MUST, Nutriscore). PA und Risikogruppen wurden mit dem Korrelationskoeffizienten nach Spearman untersucht. Die Mittelwerte der beiden Gruppen wurden mit dem Mann-Whitney-U Test untersucht und mit der Receiver-Operating-Statistik ein Cut-off Wert berechnet. Ergebnisse: NRS und MUST korrelierten signifikant negativ mit dem PA (r= -0,474, p=0,008; r= -0,472, p= 0,019). Der mittlere PA der gefährdeten Patienten war signifikant niedriger als in der Gruppe der nicht gefährdeten Patienten (NRS: PA= 4,5° vs. 5,6°, p= 0,002). Der Cut-off Wert betrug bei NRS 4,6° (AUC= 0,798) und bei MUST 4,5° (AUC= 0,617) Diskussion: Ein PA von 4,6° deutet auf ein vorliegendes Ernährungsrisiko hin. Der PA aus der BIA ist folglich ein nützliches Instrument bei der Identifikation von onkologischen Patienten mit dem Risiko einer Mangelernährung.

Background: Patients with head-neck-cancer are at high risk of being malnourished. This is due to tumour site and side effects of radio(chemo-)therapy on the upper gastrointestinal organs. This has a negative influence on the patient’s prognosis, which is why an early detection of an impaired nutritional status is much needed. Low phase angle (PA) from bioelectrical impedance analysis (BIA) is associated with nutritional risk and decreased long-term survival. Aim of this study is to evaluate phase angle from BIA as a tool in the detection of a nutritional risk compared to NRS, MUST and Nutriscore. Methods: PA in all patients (n= 30) was determined with BIA. Patients were classified according to nutritional risk (NRS, MUST, Nutriscore). Correlation coefficient (Spearman) was calculated. Median PA of the two groups was compared with Mann-Whitney-U test. Cut-off values were assessed by receiver operating statistics and area under curve (ROC-AUC). Results: NRS and MUST showed significant correlation with PA (r= –0,474, p= 0,008; r= –0,472, p= 0,019). Medium PA of patients at nutritional risk was significantly lower compared to the other group (NRS: PA= 4,5° vs. 5,6°, p= 0,002). Cut-off values were 4,6° (NRS: AUC= 0,798) and 4,5° (MUST: AUC= 0,617). Conclusion: PA= 4,6° indicates nutritional risk. PA seems to be an appropriate tool in the identification of nutritional risk in oncology patients.
