Title: Optimale Vitamin D Zufuhr in der primären und sekundären Prävention bei Morbus Crohn

Abstract: 
Morbus Crohn ist eine chronisch-entzündliche Erkrankung des Gastrointestinaltrakts, welche sich aufgrund einer Kombination aus Umwelt-, Immun-, und bakteriellen Faktoren in genetisch prädisponierten Individuen entwickeln kann. Zu den möglichen Ursachen im Immunsystem zählt die verstärkte Th1- und Th17-Immunzellenantwort, welche eine vermehrte Produktion von proinflammatorischen Zytokinen auslöst. Durch die Freisetzung dieser Zytokine wird die Epithelschleimhaut geschädigt, sodass die Permeabilität für bak-terielle Antigene erhöht ist und Symptome von Morbus Crohn entstehen. Ein Vitamin D Mangel wird derzeit häufig in Morbus Crohn Patienten festgestellt. Deshalb wird disku-tiert, ob Vitamin D einen Einfluss auf die Entstehung und auf die Prävention von Morbus Crohn ausübt. Vitamin D kann in seiner stoffwechselaktiven Form, 1,25-Dihydroxyvitamin D, durch die Bindung an den Vitamin D Rezeptor, entzündungsfördernde Zytokine redu-zieren und die Balance zwischen Th1- und Th2-Zellen aufrechterhalten. Jedoch ist weiterhin unklar, ob eine niedrige Serum Vitamin D Konzentration die Folge der Entzündung im Darm ist oder ob Morbus Crohn die Konsequenz einer Immunstörung ist, die durch einen Vitamin D Mangel induziert wird. Zu den Faktoren, die zu einem Vitamin D Mangel führen, gehören unter anderem: inadäquate Sonnenlichtexposition, inadäquate Nahrungsaufnahme, beeinträchtigte Aufnahme oder Transport im Organismus, Medikamenteneinnahme, sowie Rauchen, wenig körperliche Aktivität und ein erhöhter BMI. Das Ziel der vor-liegenden Bachelorarbeit ist es, basierend auf der jetzigen Datenlage, eine optimale Zufuhrempfehlung von Vitamin D zu untersuchen, damit ein adäquater Vitamin D Spiegel erreicht wird und potentielle positive Wirkungen eintreten. Ein optimaler Vitamin D Status, der bisher zwischen 75 und 100 nmol/l angesetzt wird, kann die Krankheitsaktivität, Krankenhausaufenthalte und das Krebsrisiko reduzieren und die Phase der Remission aufrechterhalten. Trotzdem sind weitere Studien notwendig, die die Wirksamkeit von Vitamin D bei Morbus Crohn untersuchen.

Crohn‘s disease is characterized as a chronic inflammatory disorder of the gastrointestinal tract, which is caused by a combination of environmental-, immune-, and bacterial factors in genetic predisposed individuals. Possible immune factors could be an increased Th1 and Th17 immune cell mediated response, which therefore increases the production of pro-inflammatory cytokines. Pro-inflammatory cytokines such as Tumor necrosis factor alpha (TNF-α) and Interferon gamma (IFN-γ) can directly contribute to disease pathogenesis by damaging the epithelial mucosa and increasing its permeability. Vitamin D deficiency is common among patients with Crohn‘s disease. It is discussed whether vitamin D influ-ences the development and the prevention of Crohn‘s disease. Vitamin D in its metabolic active form, 1,25-Dihydroxyvitamin D, is able to bind to a vitamin D receptor. This can reduce pro-inflammatory cytokines and maintain the balance between Th1 and Th2 cells. However, it‘s unclear, if low serum vitamin D concentrations are due to Crohn‘s disease and to an intestinal inflammation or if Crohn‘s disease is a consequence of the immune disorders induced by the vitamin D deficiency. Several factors, which influence Vitamin D deficiency, are: inadequate exposure to sunlight, inadequate dietary intake, impaired absorption and transport in organism, use of medications, smoking, decreased physical activity and increased BMI. The aim of the present bachelor thesis is, based on current evidence, to discuss the potential role of vitamin D in Crohn‘s Disease and possible levels and recommendations required to achieve them. A current optimal Vitamin D Status between 75 and 100 nmol/l can reduce disease activity, prolonged hospitalizations, risk of surgery and risk of cancer and maintain remission. Nevertheless more studies are required to research the effects of vitamin D and to establish optimal Vitamin D levels and recommendations.
